Bring completed forms to your scheduled appointment along with your insurance card and a valid photo ID.
NEW PATIENT
Before your first visit, please download, print, and complete the New Patient Paperwork. This packet is also required for returning patients who have not been seen in our clinic for over one year.
RETURNING PATIENT
Patients who have been treated at Huntsville Pool and Land Therapy in the past need to complete the Returning Patient Paperwork if it has been less than a year since the last evaluation. For returning patients who have not been through an evaluation in over a year, please download and fill out the New Patient Paperwork.
PRIVATE INSURANCE PATIENT
Patients who choose to utilize our therapy services but do not carry a preferred insurance plan may participate through our Private Insurance Program. Before your first visit, please download, print, and complete the Private Insurance Patient Paperwork.
